Record elapsed time, privileged help, undocumented steps, partial records, fidelity losses, and cleanup proof. Retest after remediation using a different operator.

Close with a reproducible recovery and remediated dependency list

Name the population, cutoff, evidence version, owner, decision, unresolved exceptions, next checkpoint, and downstream records updated. Preserve the earlier state rather than replacing it with a clean current screen.
